Patient Questions, Scripts, Referral, Letters, Forms And cancellation

Your Optimal Health Centre is open between 9.30am to 5pm, Monday to Friday, AEST. Please note our clinic closes each day between 1-2pm.

 

You can generally expect a reply within 2-3 working days, however, you may not receive a reply if your email is regarding one of the following.

 

  •  CLINICAL QUESTIONS. 
    All questions via telephone or email which relate specifically to your health care needs must be dealt with via a phone or face-to-face consultation with one of our clinicians.

    This is due to the overwhelming queries that we receive via email and for the safety of our patients, this has to be addressed via a consult through our clinician.

 

  •  SCRIPT / REFERRAL / LETTER / FORMS
    The prescription/letter/forms will be prepared after hours and a fee of $50 will be charged. Please specify in your emailed request that you are aware of this fee. We will contact you for payment of this fee prior to the script or referral being sent to you.

 

  •  CANCELLATION. Please note cancellations made with less than 48-hour notice (2 business days) will incur a cancellation fee 50% of the consultation fee as per our clinic’s policy.

 

We utilise Translation and Interpretation Services (TIS), a free service for patients who require an interpreter during their consult.  

We have access to Deaf Connect, a free service for Deaf, Deafblind and hard of hearing patients.
